Register as a Lumen Supplier
You will need the following information to complete your registration.
- Company name, DBA name, Federal Tax ID, Dun and Bradstreet number
- Year established, revenue for the last 3 years, legal structure
- NAICS/SIC codes, commodity/category codes, product/service description
- For diverse vendors, diversity certification information, ethnicity,gender,ownership, and upload of applicable supporting documentation
Diversity Categories
A minority-owned business enterprise (MBE), a non-ethnic woman-owned business enterprise (WBE), a service and/or disabled veteran-owned business enterprise (DVBE), a disabled owned (DOBE), Lesbian, Gay, Bisexual, and/or Transgender owned Business Enterprises (LGBTBEs) is a for-profit enterprise, regardless of size, physically located in the United States or its trust territories. Minority, LGBT, woman, disabled, service disabled veteran ownership and/or controlling interest must be 51 percent to qualify, whether the business is publicly or privately held OR is “minority controlled” by 40% of its voting stock, board of directors or senior management and possess 3rd party certification from the Small Business Administration (SBA) or Federal, state, local governments or councils and chambers of commerce.
2nd Tier Reporting
All prime suppliers with an average yearly spend of $700,000 are encouraged to have an MWDVBE subcontracting plan as part of their Sourcing contract and report on payments made to diverse vendors. Tier II spending consists of payments made to minority, service disabled veteran or non-ethnic women owned businesses for the purchase of goods and services, used directly or indirectly, in support of Lumen.
